• "Every revolution has its face", said a leader comment, noting that in Iran's case in recent times the face was that of a young woman shot in the chest during a demonstration (Syria: The truth will out, 2 June, page 32). This was the latest of numerous Guardian slips since 2009 in naming Neda Soltani as the woman killed that year during Tehran's protests over disputed presidential elections. She was actually Neda Agha-Soltan. Confusion in 2009 led to the name of a different individual, Neda Soltani, gaining wide circulation – to such an extent that the latter felt impelled to leave Iran. Other Guardian mentions of Neda Soltani in 2011 include a 1 June reference on our Middle East Live blog; and a reference in an online technology piece – Google to release Chrome, Earth and Picasa downloads for Iran, 19 January. In a news analysis piece about Tunisia on 15 January, we also introduced a different wrinkle, Neda Agha-Soltani (A wake-up call for the region, 15 January, page 3).

• Falkirk was given as the location of the Zaha Hadid-designed Maggie's Centre for cancer care. That should have been Kirkcaldy (All aboard!, 2 May, page 19, G2).

• Homophone corner: "Holden's full green face paint for her role of Princess Fiona nicely complimented the bright orange phizog of the returning super-judge Simon Cowell" (TV matters, 2 June, page 27, G2).
